Welcome to our Online Application. We are pleased that you have selected the Pennsylvania Distance Learning Charter School as your choice for quality education.
During the application process, you will be asked to provide basic information about yourself and your student(s). Any information you provide is confidential and will be used only for the purposes of enrollment.
Before proceeding, please note the following:
• The person completing the Online Application must be the parent or legal guardian of the student(s), and you will be asked to provide proof of custody for any student(s) for which custody has been awarded by a court of proper jurisdiction.
• Students enrolling in PDLCS must be 5 years old by September 30, 2008 and must be under the age of 22 on August 25, 2008.
• Students who are 18 years of age or older and registering as their own guardian must prove that they are financially independent (student must provide a copy of a mortgage statement in the student's name).
• Completing the Online Application does not enroll your student(s) in our school. When you have finished the online application, you must print a complete set of forms for each student, sign each page that requires a signature, and fax or mail it to the school. You must also provide a proof of residence, a transcript or grade card, a birth certificate, and an immunization record for each student.
• You must have a valid and active email address to complete the online application.
